About VivifyCare
VivifyCare are dedicated to empowering adult and older adult NDIS participants by providing tailored assessments and intervention services that enrich their daily lives through meaningful occupations.
Our holistic approach focuses on understanding each individual’s unique needs and aspirations, ensuring that our interventions not only enhance their functional abilities but also promote independence and well-being.
By fostering a collaborative environment, Vivifycare strives to create personalized strategies that engage our participants in activities they value, ultimately helping them lead more fulfilling and enriched lives.
This led us to the name 'Vivify' which simply means to enrich lives.

Meet Shannon
The founder of VivifyCare, Shannon, graduated from Auckland University of Technology in New Zealand and has 15 years of experience as an occupational therapist, including over 5 years focused on the NDIS. Shannon is AHPRA registered, a member of the OTA, and holds a working with children check and NDIS worker screening check.
With a deep passion for enhancing the quality of life for individuals facing various challenges and disabilities, Shannon has dedicated her career to developing innovative solutions that promote independence and well-being. Her special interest lies in working across a broad spectrum of disability, including neurodevelopmental, neurological, acquired brain injury, psychosocial, and physical disabilities in both adolescent and adult populations, particularly within the community setting.
Shannon combines her extensive clinical expertise with a strong commitment to compassionate care. Her leadership experience in managing large, multidisciplinary allied health teams across diverse clinical and non-clinical settings has enabled her to guide many allied health professionals and participants toward achieving positive outcomes. This has included managing complex service needs and providing tailored support to individuals and families.
Her dedication to enriching the lives of individuals with disabilities has been instrumental in shaping VivifyCare’s mission to empower individuals, families, and communities, ensuring access to essential resources and support for everyone to thrive.
Frequently Asked Questions
We accept NDIS Self-managed, and NDIS Plan Managed funded participants.
We also accept private clients.
Occupational therapy assessments and intervention for NDIS clients. This includes:
- Functional Capacity Assessments
- SIL & SDA assessments + Transition planning
- AT assessment and reports (basic and complex).
- Capacity Building
- Basic home mods (rails, ramps etc) to improve access and support needs
- Telehealth
VivifyCare is a community mobile service. We can provide services to the Sydney Metropolis. This includes Eastern Suburbs, Inner West, Sutherland Shire, North Sydney and Western Sydney.
